Insurance
We accept most insurances. Our team submits insurance claims for all office and surgical services. As a courtesy, we also file secondary insurance claims. You must present insurance cards for and notify us of all medical insurance policies you possess and let us know about any changes to your insurance policy prior to your visit with us. You handle:
- Co-payments at the time of service
- Deductibles or coinsurance
- Services not covered by your insurance company

Patient Statements
We include all your charges in a single monthly statement. Even though your insurance company might cover the charges, we will continue to send you statements until you’ve paid everything.
Payment Methods
You may pay your bill by mail, over the phone, or in person. We accept cash, personal checks, Visa, American Express and MasterCard.
Payment Plans
If you have an outstanding balance after your insurance provider’s contribution and cannot pay the total within 30 days of service, you may pay in monthly installments.

Understanding Your Insurance
If your insurance does not have our facility listed as a provider, please call our office and speak with the insurance department with any concerns or questions.
It is very important for you to be aware of the requirements of your health insurance policy so that you can save out-of-pocket expenses and frustration later. Our staff does its best to help, but we cannot know everything about the hundreds of insurance plans available to our patients. If you have questions about your benefits, call your insurance company’s member services department or go to your employer’s human resources department.
Referrals
Some patients may need a referral/authorization from their primary care physician before they see a specialist or go through testing. It is your responsibility to get the referral/authorization, if required, for your visit. Failure to do so could leave you responsible for charges. Most times, the insurance company won’t allow your primary care physician to allow the visit retroactively.
If your physician gives you a written order for testing or refers you to a specialist, don’t assume that your insurance plan has cleared these services. Your physician may not be aware of your insurance requirements.
It is your responsibility to ask our staff for any paperwork that may be required. Please remember to bring a written referral or order form with you to your appointment if you receive one.
Office Visits
Payment is due at the time of services, except for Medicare or Medicaid patients. If your insurance requires a referral from your primary care provider, it’s your responsibility to bring it to your appointment, or you will have to reschedule. We will charge you a $25.00 fee for appointments canceled without 24 hours’ notice.
Surgery
We will call your insurance company to verify coverage and pre-certify your surgery. As a courtesy, we can file your claim with your insurance company. You will be responsible for your deductible and the co-insurance that your insurance has not met and covered. We will need to collect this amount before your surgery.
If you do not have insurance, we will give you an estimate of the surgery charge, which is due before your surgery.
Please note: Our bill is for the surgeon’s fee only. You may also receive bills from anesthesiology, pathology, laboratory, radiology, the hospital, and sometimes consulting physicians or assistant surgeons.
