BILLING:

We accept all insurances. We will submit insurance claims for all office and surgical services. You are responsible for:

  • Co-payments at the time of service
  • Deductibles or Co-insurance
  • Non-covered services by your insurance company

As a courtesy we also file secondary insurance claims.

How to pay:

You may pay your bill by mail, over the phone or in person. We accept cash, personal checks, Visa, MasterCard, American Express, and Discover.

Patient Statements:

Your charges will all be included in a single monthly statement. Although your insurance company may cover and pay for the charges, you will continue to receive statements on your account until all charges have been paid.

Payment plans are available:

After your insurance has paid and you have an outstanding balance and you are unable to pay the total within 30 days of service, you may arrange to pay in monthly installments, which would be due at the same time each month. Contact our billing and insurance department at (409) 670-9200 to arrange a payment plan.

Understanding Your Insurance:

It is very important for you to be aware of the requirements of your health insurance policy so that you will save out-of-pocket expenses and frustration later. Our staff tries to be helpful to you, but we cannot know everything about the hundreds of insurance plans available to our patients. If you have questions about your benefits, call your insurance company’s member service department (listed on the back of your insurance card) or go to your employer’s human resources department. Managed care plans, in particular, have many rules that you and your physician must follow. For example, you will most likely need a referral/authorization from your primary care physician (PCP) BEFORE you see a specialist or have testing done. IT IS YOUR RESPONSIBILITY TO OBTAIN THE REFERRAL/AUTHORIZATION, IF REQUIRED, FOR YOUR VISIT. Failure to do so could leave you responsible for charges. In most cases, the insurance company will not allow your PCP to authorize the visit retroactively. When your physician gives you a written order for testing or refers you to a specialist, DO NOT assume that these services have been cleared through your insurance plan. Your physician may not be aware of your insurance requirements.

You must take the responsibility to ask our staff for any paperwork that may be required.

When you are given a written referral or order form, remember to take it along to your appointment.

If your insurance does not have our facility listed as a provider please call our office and speak with the insurance department with any concerns or questions.

To help you to spend less time in the waiting room, we made a copy of the forms you will need to fill out prior to your office visit.

Our Policy

OFFICE VISITS:

Payment is due at the time of services except for Medicare or Medicaid. If your insurance requires a referral from your PCP it is your responsibility to bring it to your appointment or you will have to reschedule.

SURGERIES:

We will call your insurance company to verify coverage and pre-certify your surgery. Out of courtesy we will file your claim to your insurance. You will be responsible for your deductible and the co-insurance that has not been met and is not covered by your insurance. THIS AMOUNT WILL BE COLLECTED BEFORE YOUR SURGERY.

If you do not have insurance we will give you an estimate of the surgery charge, which will be due before your surgery.

OTHER CHARGES:

Our bill is for the surgeon’s fee only. You will also receive bills from anesthesiology, pathology, laboratory, radiology, the hospital and sometimes consulting physicians or assistant surgeons.
